What is a fractional CO₂ laser?

A radiofrequency-excited CO₂ laser on a mobile cart. It splits its energy into microbeams. Each one treats a spot about the width of a human hair, down to 120 microns, and the power per spot is adjustable to the patient in front of you. Multiple scanning modes and four interchangeable tips cover everything from a superficial pass to a surgical one.

Early fractional CO₂ for surgical scars

Published studies show that surgical incisions treated with fractional CO₂ within the first month of healing had significantly better scar outcomes than untreated controls. Some studies began treatment on the day of suture removal. Treatment no longer has to wait until the scar has matured.

A practice that closes incisions and sees the patient again at suture removal, from Mohs and facial plastic surgery to breast and thyroid work, can offer early scar treatment at that visit. 02 · Carbon Wave

A new treatment on a laser you may already own.

Carbon Wave is a carbon-embedded hydrogel patch. Put it on the skin, fire a Q-switched Nd:YAG at it, and the patch converts that light into a mechanical shockwave. No new capital equipment.

How it works

A Q-switched Nd:YAG is a different physics from a CO₂ laser. Instead of removing tissue thermally, it fires ultra-short pulses that shatter a target mechanically and leave the surface intact. It is commonly used for tattoo and pigment work. Hyperion Medical sells the patches and the compression optic; the laser is the one already in your room.

Q-switched lasers work photo-mechanically: the energy is absorbed and a shockwave is produced. Normally the target is tattoo ink or a melanin aggregate. The patch gives the laser an exogenous chromophore at the surface instead, so the shockwave is generated where you want it and directed into the tissue below.

A compression optic sits on top of the patch and contains the wave rather than letting it dissipate. Bench pressure went from about 12 MPa to roughly 40 MPa.

The mechanism has a name in the literature: acoustic subcision. The comparison usually drawn is lithotripsy, where a shockwave fragments something dense without tearing up what surrounds it.

What the histology showed

In an ex vivo porcine skin study by Martin Kassir, MD, FAAD, treated samples showed clear fragmentation and disorganization of fibrous tissue on hematoxylin and eosin staining. Control tissue kept its native structure.

This was an ex vivo model, not a clinical trial. The study's own conclusion is that further in vivo research is needed to optimize parameters and confirm clinical efficacy. Ask us for the full study and the ROI model, and we'll send both.
